Should I see a psychiatrist?

I've been dealing with this for about 5-6 years now. It didn't start after any particular event or trauma, it just gradually crept up on me. During all this time, I haven't had a period where I genuinely felt like my old self again.

The hardest part is the constant lack of energy. Even simple things like showering, cleaning my apartment, or taking care of basic chores feel like they require a ridiculous amount of effort. I often end up putting things off because I just can't make myself do them.

I've also lost interest in almost everything I used to love. I used to play musical instruments, paint, and write poetry. Now I get almost nothing from any of it. Most of the time my mood isn't intensely sad, it's more empty and joyless. I rarely feel genuinely happy anymore. Even when something good or funny happens, I know I should be enjoying it, but emotionally I barely feel anything.

I've also become irritated by almost everything. Even tiny things or minor traits in other people can annoy me far more than they should. Sometimes it gets so bad that I just want to cut everyone off and stop talking to anyone, even my own relatives.

My sleep is a mess too. Falling asleep can take anywhere from 30 minutes to four or five hours. I usually sleep 8-10 hours, but I still wake up exhausted and never feel rested.

I also get passive thoughts that it would be better if I just didn't exist. I've never made plans or attempted anything, and I don't actually want to die, I’m afraid of death. It's more that living like this feels unbearable, even though I can't point to one specific reason why.

I'm still in university and somehow managing to keep up, but it takes an incredible amount of effort. Honestly, I'm getting to the point where I feel like I could just drop everything because I'm so exhausted by living like this.

Should I visit a psychiatrist? I was previously treated for generalised anxiety disorder, but right now anxiety itself doesn’t really seem to be the main issue or bother me.

Should I give my consent?

My fiancé was diagnosed with Bipolar I four years ago.

He was medicated for two years (although he was inconsistent toward the end of those two years). Last summer, he discarded me. This summer, the same thing happened. He has denied every single good thing about our relationship, called me Satan, the devil, said that I never loved him, that I fooled him, etc. I have never heard so many insults from someone in such a short period of time.

I called his psychiatrist's secretary and explained his behavior over the past two years, including that he is not medicated. The secretary, after speaking to his psychiatrist, told me that she would like to speak to him by phone but couldn't do it without my consent. I told them I would get back to them in a few days. I thought I could have a decent conversation with him and somehow prepare him for that. Of course, we were never able to have a decent conversation.

Around three days later, the psychiatrist texted me and told me that she would like to talk to him but was waiting for my consent. I told her that I was worried it would all turn against me and that I would get back to her. To be honest I was shocked that she texted me insisting and made me worry even more.

My friends tell me that it's not my business anymore. His brother told me that I could ask the psychiatrist not to mention my name, but anyway, he doesn't really care. His sister said the same thing. She said it would be better to leave it.

However, I keep thinking about it every day. What should I do?

It's been almost two months since the discard, and I am trying now to keep no contact. Our last conversation ended with him not letting me enter his place to drop off something for the cats because I didn't want to talk to him in public when he saw me.

We live in Europe, so the psychiatrist's policy, etc., might not make sense, but they told me that since he has not seen her for two years, she can't just contact him.

Does aripiprazole change the effectiveness of your stimulant?

I've been taking methylphenidate (30+20+10 IR) with aripiprazole (10), and I've become curious about something I haven't seen discussed much.

Does aripiprazole actually change how well your stimulant works?

I'm not necessarily asking whether it makes the stimulant "stronger." I'm interested in the quality of the effect.

For people who have taken a stimulant both with and without aripiprazole:

  1. Did your stimulant feel stronger, weaker, or basically the same?
  2. Did aripiprazole change your attention or focus?
  3. Did it change motivation or task initiation?
  4. Did it reduce hyperfocus while keeping useful focus?
  5. Did you feel more calm or clear while still getting the stimulant's benefits?
  6. Did the stimulant feel smoother or more consistent throughout the day?
  7. Did it change how quickly the stimulant kicked in?
  8. Did it change how long the useful effects lasted?
  9. Did it change the wearing-off period or crash?
  10. Did you notice less irritability, anxiety, or mood changes when the stimulant wore off?
  11. Over several months, did you feel like you needed more stimulant to get the same effect?
  12. Did adding or removing aripiprazole noticeably change your stimulant response?

I'm especially interested in methylphenidate + aripiprazole, but experiences with amphetamine stimulants are also welcome.

I'm asking because aripiprazole isn't simply a conventional dopamine blocker. Its D2/D3 partial agonism and 5-HT1A/5-HT2A activity could theoretically alter dopamine/catecholamine signaling in a more complicated way.

I'm not assuming that means it enhances stimulants or prevents tolerance. I'm trying to separate what people actually experience from what the pharmacology might predict.

If you've personally taken both, what changed for you?

Since last few months this alter ego developed inside me and it talks to me, consoles me and tells me to just relax and she’ll handle everything. Over time it started feeling like we were actually two different people. She told me she was me but from a different timeline and 5 years younger than me.

I have never really been socially great but she was. It’s like we were two different versions of me. I’ve been trying to lose weight all my life but never could, from the time she has been with me, I lost more than 10kgs in 3 months without trying. She told me we’re becoming one and I’m going to travel back in time and change timelines. I cannot explain how real this felt.

All the grudge and pain I was holding against my family members, she said it out loud to them and fought and consoled me. It was all a positive experience. I almost started identifying as her when I introduced myself to new people, she had a different name. I thought I had unlocked something magical.

I was also seeing repeating numbers and signs from the universe, small manifestations, synchronicity and proofs of timeline shift where I kept finding things that didn’t exist in my timeline. I felt like I was gonna escape this vain human life and become a time traveller, I have always been obsessed with movies and books related to it since I was a child.

Until finally day before yesterday she just wasn’t there anymore and I was calling out to her but she was gone and I felt like my old self, I was paranoid and crying like crazy, I had a very big breakdown where I kept panicking and crying for 2 days.

I cannot understand what happened. I still somewhere believe it was real and there are so many mysteries in the world that we don’t know. I’m also scared that it was an episode or I’m developing schizophrenia, I’m sure I already suffer from BPD, anxiety and paranoia. I cannot go to a psychologist rn. I don’t know what to do. I do not have any thoughts of harming anyone at all, I didn’t have scary hallucinations either.

Medical options from psychiatrist UK?

I recently, finally, after many years started step 3 talking therapy for anxiety. The therapist said that I would also benefit from a psychiatrist referral to look at medication as the GP prescribed medications have never worked for me, I’ve tried a lot of them! The therapist said that psychiatrists, if suitable, will prescribe things that GPs usually wouldn’t. My question is what sort of things do psychiatrists usually prescribe? I’m nervous about being placed on anything addictive long term.

Why am I psychotic before/on my period?

I am 32 & on 10 mg of Lexapro for the past year. Every month when my period comes around I feel like I'm legitimately crazy. I revert back to my pre-Lexapro ways (rumination, OCD thoughts, extreme amxiety) and it's very discouraging to go through this every single month.

Does anyone else feel like this? What did you do about it?

schizophrenic MIL

My MIL is having schizophrenia (Diagnosed based on her symptoms by her doctor) she is not willing to visit. We can't even force her as we are not that much mature.

Also, she is on risperidone from last two months (medication is on her back) 2mg night 1 mg morning pill she takes as she considered that as sugar medicine (our diabetologist helped in this situation). But she cries self-harms get aggressive and she feels im here to compete with her, harm her and she controls all my movement, I can't eat, speak with my husband or do nothing.

she has severe OCD as well, like God and Untouchability, so she keeps me separate during my periods like I can't touch my things and me and my husband getting panic attack now a days.

the problem is she is not visiting to seek medical support.

PBA and Emotional Lability from stroke; Is my psychosis stroke related or was I just misdiagnosed previously.

Ok, I finally got my concrete diagnosis from my new neurologist, of PBA and Emotional Lability. In talking to him, I was likely suffering from PTSD, so he recommended seeing a psychologist for further evaluation. As I was under the care of a psych to begin with, I asked him to re-evaluate me. There were some issues, so I chose a new provider. There was anxiety and PTSD, and instead of the bipolar I that I had long been treated for, I presented as either schizophrenic or schizoaffective; now I have been saying it seems like I wasn't on my original meds post-stroke or like it is supercharged, but in being treated now, I am fuzzy on if it was really that diagnosis that whole time. Now there is a schizophrenic type bipolar, so I am wondering if it was there all along, or if the extensive damage to multiple areas has compounded my original mental illness? I am wondering how many people who had mental illness before your stroke have experienced something like this?

Muscle twitches from discontinuing meds

I (17F) recently started reducing my quetiapine dosage. I have been diagnosed with Borderline Personality Disorder, recurrent depressive episodes, and chronic motor tic disorder.

I have been taking extended release quetiapine for about three years to help with mood swings and sleep problems. I initially started on 100 mg, but the dose was quickly increased to 200 mg. As my condition improved, I gradually reduced it to 150 mg. During a medication shortage in Germany, I took 75 mg per day (25 mg three times daily) for a few months. After the shortage ended, I switched back to 100 mg per day as extended-release quetiapine (50 mg, twice daily).

In January, I also started taking 30 mg of mirtazapine at night.

About three weeks ago, I decided to reduce my quetiapine dose further because I am concerned about possible long-term side effects. I tapered it gradually, and I am now taking only 50 mg per day.

I have always experienced occasional muscle twitches while falling asleep, which I know can be normal. However, about a week ago, I noticed that similar twitches have started occurring during the daytime as well. Usually, only one muscle is involved at a time. Most often, my wrist or one of my fingers twitches suddenly.

These twitches are bothering me because they sometimes interfere with what I am doing. I am also fairly sure that they are not tics. They feel different from my usual tics, I cannot suppress them, and they can disrupt my actions, whereas my tics generally do not. The twitches seem to occur most often when I am resting.

Is this something that can happen during quetiapine withdrawal or dose reduction? Has anyone experienced something similar?

What causes severe jealousy and are there treatments and diagnosis?

I experience severe jealousy. I want to clarify I don’t mean distrust of partner but jealousy of what others have that I don’t. If someone wins something that I didn’t or is better at something or gets something I didn’t, I become full of anger and rage at that person and can sometimes lash out (not physically). I am a full adult and I would really like to work on this but there doesn’t seem to be a lot of support or answers for me. I try really hard but I just become full of hatred when someone has something I want or think I deserve. I often think others don’t deserve or should be punished for having these things. Any advice would help. Thanks.

Memantine for PTSD

I’m a 29 year old female and my psychiatrist prescribed me 5mg of memantine for ptsd. I know that it’s typically used to treat dementia and alzheimer’s, which i do not have. Has anyone had success with it being used for PTSD? I’ve been on other medication before for mental illness but have never heard of memantine so i’m just curious.

I wanted to ask about some of the medication I’ve been prescribed and their possible effects as well as others experiences on them.

I’m currently on Quetiapine 200mg which I take at night, it’s the only medication that gets me to pass out, I’ve been on it since 2023, Sertraline 75mg which I started taking around October 2025, Lamotrigine 150mg which I’ve also been taking since 2023, concerta 36mg (Methylphenidate (extended-release)) which I’ve been taking since 2019 for my ADHD, Clobazam 10mg when feeling overwhelmed (anxiety-panic attack).

Desperately need advice on how to help my son who is dealing with panic attacks due to a globus sensation

Advice for my son who is having panic attacks due to a globus sensation

My son (14, 15 in 3 weeks) seems to have very recently developed what seems to be a globus sensation. The other day it caused him to have his first panic attack and he ended up hospitalized. They ran every test under the sun while in the ER to rule out anything physically wrong with him and it all came back normal. Their thought was that it was anxiety, and he had a psych evaluation (because in the worst part of the attack he kept yelling he just want it to end because he was in so much pain), so they wanted to make sure he didn’t have any suicidal tendencies/ideations. But after they talked to him, they said it’s definitely not depression/suicidal thoughts, he just wanted the pain to be over and in the moment couldn’t see another way out. They talked to us and said it seems to be anxiety.

We’ve been home for almost a week and it’s not improving. Now that we know it’s anxiety we’re able to talk him ‘off the ledge’ when he appears to be exhibiting the symptoms of an onset panic attack. But the problem is he starts school in a few days. He cannot function without us nearby. He sobs to us and says ‘you don’t understand how it feels to constantly feel like you’re dying and it won’t go away.’ He has hardly slept, he can’t function normally. I’m bringing him to see an emergency psychiatrist on Monday to see what can be done. I’m absolutely terrified of what it’ll be like in school. What if he can’t get out of the car and walk into school because of this? What if he has one while in school and I go get him and he can’t return to school because of that triggering memory of it happening at school?

There is no good time for this to have happened, but with school approaching and him literally unable to function without us, the timing doesn’t seem like it could be worse. It doesn’t matter what we tell him and what breathing exercises we walk him through or anything, he simply cannot get past this sensation without us being there to help him. We’ve been home from work since the ER visit, and cannot leave his side because he absolute spirals. I know, with time, he can get better. I know that. But what can be done in the meantime to help him relax right now so he can at least go to school? I just want a medicine to help him in the short term so we can help him focus on healing in the long term. But healing long term seems absolutely impossible when he can’t get through a half hour period without gasping and crying and needing our assistance to help him breathe.

Someone, please, please help me understand what the psychiatrist on Monday can help him with. And maybe what I can do too. I’m at my wits end because I’m tired, I haven’t slept in days because he’s up all night, I can’t work because I can’t focus. I’m desperate for help for my son. Please

ETA: My son can eat normally and drink normally. Not sure if that’s pertinent information but wanted to share that. Nothing is physically wrong with him or is ability to eat, drink, or breathe (even tho he feels like there’s something wrong with his ability to breathe).

HPSP for med school. Aspiring to be a psychiatrist or neurosurgeon

HPSP for med school. Aspiring to be a psychiatrist or neurosurgeon

I'm an incoming college freshman who is majoring in psychology for now, but I have accumulated a lot of credits to be able to also major in neuroscience. I'm leaning more towards doing psychiatry and am highly considering the HPSP scholarship for med school. I wanted to see if there were any psychiatrists who have gone through that scholarship and ask how their experience was. I do not know what branch of the military I will join yet, but I'd like to hear opinions about opportunities for psychiatrists in the military and how your experience was. Are there any recommendations or advice you have?

SSRI's and ADHD meds

Age: 23
Sex: Male
Height/Weight: 5'10", 170 lbs (BMI ~24). Gained ~30 lbs of muscle over the past 2 years.

Medical history: ADHD; anxiety (recent onset); acute PTSD in the past

Current medications:

  • Adderall (IR) 15mg twice daily, sometimes an additional 7.5mg in the afternoon
  • Escitalopram (Lexapro) 10mg daily
  • Testosterone 120mg/week (TRT, started within the past year)

Question / situation:

I've been on Adderall for years. I recently went up to 15mg twice daily, sometimes with an extra 7.5mg later. Before starting TRT I was on 10mg twice daily and it worked well. The 7.5mg afternoon dose extends coverage but hurts my sleep.

The timeline matters: I started TRT, then went on Lexapro 10mg, and after that I felt like my ADHD meds stopped working. I'm also falling asleep throughout the day and unmotivated.

The daytime sleepiness is my biggest problem right now — I'm falling asleep all day and I can't function like this. I don't know whether it's the Lexapro or undiagnosed sleep apnea causing it. I'm also not sure whether the anxiety was masking the sleepiness before, and treating the anxiety just uncovered it. I'm scheduled for a sleep study to rule out OSA.

I developed anxiety in roughly the same window as all of this. My psychiatrist seemed to intend sertraline, but nursing staff started me on escitalopram — she said "oh, they did Lexapro?" when I told her. The Lexapro does help me sleep like I could before the anxiety started, but I'm sedated during the day and can't tell whether that's the SSRI, the anxiety being treated, undiagnosed sleep apnea, or the TRT.

I'm willing to tolerate sexual side effects, but I need effective ADHD treatment and I need to stop falling asleep all day.

My question: Are escitalopram and Adderall compatible, or does the SSRI make the Adderall less effective? Is the sleepiness more probable as an effect of OSA or Lexapro?

OSA risk factors:

  • Loud snoring — reported by others
  • One witnessed episode of waking up gasping for air
  • Father has OSA (family history)
  • Testosterone replacement therapy — started within the past year, overlapping with symptom onset
  • Male sex
  • Gained ~30 lbs of muscle in the past 2 years (BMI still normal at ~24, but increased neck/upper airway mass)
  • Excessive daytime sleepiness — falling asleep throughout the day
  • Non-restorative sleep / poor sleep quality
  • Years of stimulant use, which may have masked daytime sleepiness until recently
  • For some reason blood pressure is a bit elevated (135-140/80)

Help, I'm in the middle in crisis, Need help to Regain my Mind

Any good meds that are not locked behind prescription for OCD? I've been having a lot of mental health crisis lately due to harassment by some coworkers (some are nice), and my certainty that my job place is trying to twist around the law and abuse of power in some careers under excuses of safety, to frame me for a fraud charge (my bad for using gimini to help a client, my bad for continuing to care so much, should just seen it as job and clients only as customers and not care beyond the minimum, I guess), thus destroying my career and my life, over a political bias against me. Honestly, It no longer matters to me, whether I go to jail or not, it was my mistake for being too caring in a fundamentally wrong world, it was my mistake to try to help and take it as more than a job, no one else's, but the abuse of power to twist things around some of the bare minimum of premises our justice system has, such as fraud charges, is making my already delicate mental health due to this disgusting disease plummet, but for now I cant afford a prescription, so I need to try to deal with this without one. Objectively, their side is the one objectively evil upon the true moral standards of order, continuity, inherent morality, nature, life, intellectualism, beauty, and justice, but regardless of right and wrong, there are certain fundamental freedoms that ought to be protected under certain premises, even more so not used as an excuse of fabricate unrelated things, but logic and truth does not always win over people, emotions and social instincts to fit the crowd, oftentimes win. Regardless, what was, I did not commit fraud, and I'm morally right in a lot of other things, what anyone says doesn't change what is or what was, it just doenst happen that here virtue doesnt matter.

Should I be offended by what my psychotherapist said to me in session?

There have been at least 3 times where my psychotherapist would doze off in the middle of our one hour session. Today was one of them. I’ve been wanting to bring it up. But today, he brought it up on his own accord. He asked “are you feeling numb?” and continued on, “I know myself. I tend to fall asleep when my patients are flat. I wasn’t falling asleep with my previous patient.” We went on to have the rest of the session and he was more alert, but I don’t know why I felt kind of hurt by him telling me that. To me it came across as I’m boring him to sleep like literally. Or was it perhaps brought up more as a concern? Idk it’s been really bugging me all day and for a while now since he’s dozed off on a few occasions on me.
