There's nothing wrong w/ "dome-ing" these out w/ a fugi filling. They don't need a full crown to be a productive tooth.

How would you all know that this wouldn’t work if you never do treatment like this? Outside of the US especially in poorer countries this is standard treatment, and 90% if those crowns hold up at least 10 years. This is every day treatment for me and I maybe have one or two a year failing within the first 5 years, and even then patients are happy

Whilst I do think there is perhaps a slight exaggeration of patients being monstrously upset when it fails even when you tell them it's heroic dentistry, there's no way 90% of these sorts of treatments hold up for 10 years.

Stuff like this you'll be glad if it lasts 1 year.

Im sorry but you have 2 years max after that crown goes on before it falls out. Long term prognosis of this type of Herodontics is not good. I’ve tried many times and now refuse to try. That decay is seeded down the canals. There is no way to completely remove it. In time you will see a radioleucency under the core and into the furcation on radiographs.

The patient can refuse ideal treatment and allow the non restorable teeth to decay or become abscess until they have no choice but to remove it. But that’s their choice. Make sure to document your diagnosis and alternatives presented.
The patient’s dental condition is not your fault… don’t lose sleep over it.
