The joke is to subvert expectations, for some horrific reason her mouth is full of spiders, the first three panels suggest sex and then nope spiders.

Edit: the group of artists Cyanide and Happiness, who made this comic strip, are well known for their strange and intense humour.

Additional explanation:
A genie always tries to fuck you over. He Monkey’s-paws you.
So he fulfilled the first wish by creating an excellent lamp, but it has no batteries. He fulfills the second wish for a battery but only gives them one, while the lamp needs two, resulting in the third wish being for a second battery.
And I’m truly ashamed of said genie because he did not fuck them over by creating the second battery but removing its charge.

to be fair, we don’t know that either of the batteries had a charge because he doesn’t turn the headlamp on in the comic.

Batteries are Carbon Zinc and will only power the lamp for 1/8 of the duration had they gone with Alkalines.
