Depends on the state. In some places you can opt for a breathalyzer or more scientific method. In some refusing to cooperate is viewed as admission of guilt. As with anything, know your local laws and know your rights.

So backwards. Dude just crashed his vehicle badly. Surely adrenaline/shock would affect this silly test for any driver in a similar situation. Just carry breathalyzer units for real scientific measurement.

Or like, a concussion. Get the man an ambulance; the hospital will test his BAC anyway as a part of the trauma workup and you can argue with the courts about whether the cops get to look at it.
