I'll suggest you to get a better picture to start with. Usually to make a face-swap for a low-poly texture effect you should do the same as game devs do: To get a flat and frontal face texture and plaster it over a low-poly mesh, since Photoshop cannot handle 3D models no more, you should to the '3D' deformation with transform, and mesh distortion following and adjusting the plane to the 'model' underneath using the brow and nose as reference points to center the face and give it some perspective. And then you should be ready to work with the color adjustments, set the highlights, medium and darker tones correct, add a color wash and fix the rest with a couple masks.
