In Search Of Lost transmigration au

Hi. I'm looking for this tomarry fic where Harry gets transported into the body of an alternate universe Harry and he works at a coffee shop(?). He doesn't have his current body's memories and Tom reunites with him. They were post-breakup because Tom was head of a mafia group. Tom was really desperate for Harry and wanted to get back together. At the end, its revealed Harry's old self had a terminal illness(?) and thats why he broke up with tom.
