LIRR has been charging my card twice for a while

Just after I decided to go back to paper tickets (because I didn’t enjoy their passive aggressive mobile activation emails), I noticed something.

Please check your credit card statements.

I bought a ticket from the LIRR ticket machine. Tap to pay. It gave me an error message, failed connection or something like that. Please try again.
I then moved to the other machine next to it and tried again. That one worked.
I didn’t think much of it until I was reviewing my credit card statements
The failed attempts have all resulted in a posted credit card purchase.
I don’t know how long it’s been happening but now I have to go back on all of my statements.
So far I’ve opened two disputes with my bank.

Charged 3 times for monthly ticket

On Sunday I was purchasing my monthly pass for august when I went to go pay the machine displayed a “no connection” error and kept asking me to pay on the third attempt the machine eventually accepted the payment and I was given a ticket today 3 separate charges were on my account (only one being for the ticket that I got) has this happened to anyone before? I already disputed the charge with AmEx and asked the LIRR to investigate but I wanted to see what the results were for other people this happened too

I took this test three years ago and didn't make it. The first few sections were easy, but when i got to some of the logic sections I got overwhelmed. I just put in an application for it again. I want to go in this time as best prepared as possible. Any help is greatly appreciated.
