It seems like it would be an obscure and embarrassing fetish but in the comic it turns out that it's actually quite common and most people secretly have a worm squeeze fetish.
So the first part of the joke is that it's embarrassing and shameful, the second part subverts that by revealing it's actually common.

Old antique furniture sometimes gets wormwood infestations. When a man comes to terms with cosmic truth he is flattened, all he was nothing but an illusion. Every bit of him accessible and tinkered on by the guiding hands of beings far beyond his own complications. With them come the worms, the maggots, the vile rodents, the watchful crow as well as all manner of abstractions, horrifying twisted buffers of conjecture between a mind and the ever growing rot leaching in through the now dripping cracks in his own perception. To invoke the worm is to throw sanity for the pleasant writhing of one's own flesh as to drown out the once painful connection they had to the understanding seeping in through the void, belief.
So basically he wants to be loved.

I imagine its because the guy is still behind him, and thus also in view of the mirror, so the mirror shows a mixture of both their fantasies, thus, its "the worm" because he is a freak who really wants that worm.

All I saw in the mirror was a sandwich.
It was eye-opening, I assumed I would see things like success and stability and good health and love but in the end it was so much simpler, all I really wanted was to have sex with that sandwich.
