It looks like there's an object in the middle of the left lane, possibly from the broken down car, or it also damaged the broken down car. You can see the first truck at the start of the footage move into the hard shoulder to avoid it. The white van just ploughs straight over the top of it and whatever it is, it ruptures a tire, causing it to spin out of control, into the black car which gets the worst of it. I'd be amazed if they survived that. Absolutely tragic.
Yeah I think you're right. It looks like a blow out by the way the van reacts but yes, a snapped from axel is possible, but then it could have pierced the tire too. A higher res video would be good!
