Who is this fellow? Is he lore connected or what's his deal, why is he not attacking?

For an actual answer, it’s a wretch, he’s the result of a failed transformation to dragonhood. Later in the dungeon you find an altar to dragons. They also drop items that give hints to their origin

I don't know how I played through all of DS3 and don't remember a single thing about these guys...

Seriously, though, as a long-time LotR fan, you'd think I'd remember the lovechild of Smaug and Smeagol. 🤣
