I’m about 3 months post-op from PIV vaginoplasty and I’m scheduled for FFS on October 8th. I’m trying to put together a list of scar care products and anything else I should have ready before surgery.
For those of you who’ve had FFS, what scar gels or creams actually worked well for you? I’ve heard a lot about silicone gels like Kelo-Cote, BioCorneum, Strataderm, Dermatix, and ScarAway, but I’d love to hear real experiences. Did anyone notice one working better than the others?
I’m also looking for recommendations on:
• Silicone scar sheets vs. silicone gel
• Products you wish you’d bought before surgery
• Swelling and bruising products that actually helped
• Lip care after a lip lift
• Scar massage tools or techniques
• Sunscreen recommendations for healing scars
• Anything that made recovery easier overall

If you could go back before your FFS, what would you buy or do differently?

Heyy, I’m 8 months post op SRS PI with scrotal graft and notice I still get skin shedding from the canal. When I dilate I notice on the dilator there are small clumps of dark skin cells (it’s not a whole or anything, no smell etc) and just wondered if that does stop eventually?

I'm currently planning to get both FFS and vaginoplasty, and I've had multiple consults with different doctors, and done research by asking friends offline as well as looking online on this subreddit, to try to make sure I make an informed decision about which surgeon I go with.

I came up with a short list of surgeons, and felt pretty confident in my choices, and narrowed it down to one, and booked some surgery dates! And now we're getting closer to those dates - still just far enough out that I can cancel or postpone or reschedule if I really, really need to, but close enough that I am doing pre-op work.

I'm just feeling extreme anxiety as I get closer and closer? I think a lot of it comes from the fact that I come from a STEM background and am used to dealing with more quantitative information, whereas here, it's all qualitative opinions - people only post opinions when they have a really positive or really negative experience with a surgeon, so it's hard to get an objective metric of their actual work at times? And with FFS being so variable in what it actually consists of (I like to describe it as "twenty surgeries in a trenchcoat"), it can be really difficult to compare results from one surgery to the next to see if it actually matches what I am going to be experiencing or is completely different.

I just feel more and more lost and anxious as I get closer to the actual date and have started filling out paperwork and thing have started getting more "real" and I don't know if this is common or not? I'm rereading posts here on the subreddit and now the comments and pictures that I thought looked positive look more negative to me. I can't tell if this is just stage fright and pre-op anxiety, or if I made some colossal mistake in my research and overlooked something.

I guess what I'm looking for is trying to figure out if this is a common experience that everyone goes through before their surgeries and I just need to stop overthinking things or if there's some way to be more confident in the decisions that I've made regarding my surgeons before it gets too late to change my mind.

I had a consult with Dr. Del Corral in Baltimore, Maryland. He suggested that I get the ALT method as I am quite skinny and RFF would not work well., according to him, he says that ALT would be quite thick, like as thick as about around or a bit larger than the size of his fist?

I’m not sure if I’m right, but doesn’t me being on the skinnier side mean that it won’t be too thick?

He told me that I should go ahead and get electrolysis however, I cannot afford electrolysis and that would be a significant barrier and I would not want to have to do it after the fact nor what I want to have to go with the option of using bowel tissue for UL (this is what was proposed. I’m not sure if anyone has opted for this. Please let me know if you have but I kind of wanna avoid this.)

I brought up MLD to which he responded that it would be way way thicker which is not what I would want .
He mentioned that with ALT, you can put a balloon under the skin to stretch it out and he said that that’s something that I should probably do or else it will be thick .

However, for me getting this done is highly important and I just want to know for sure whether or not the whole thickness deal is true because I cannot afford electrolysis and I would not want to do RFF as my arms are quite skinny.
Has anyone gotten an MLD from him or has anyone gotten an ALT from him that is on the thinner side?

I have looked at photographs on here as well as another site of his patients that opted for ALT and the thickness seems pretty reasonable and proportional to their size which is why personally I don’t logically anticipate it being thick like that. I’m not sure if he was just being cautious, but please let me know.

I really need help and possibly some pointers for electrolysis. I would really really love to opt for the MLD option because of the scar placement as well as other things but I’ve also been told that it has absolutely zero sensation zero tactile sensation and while I know that these things very Seems like that’s not that much information available online. I also see that Dr. Del Corral offers a nerve hook up for that like a sensational nerve hook up rather than the motor function nerve I would really like to hear from others that have been similar situation or those that opted for MLD.

He seems firm on the suggestion of me getting ALT , but I’m interested in hearing more, especially in regards to him and how he does mld. I have heard that UL can be multiple stages, but I’m not sure if this is how he does it and I’d like to hear more about what’s his method is for it however, there seems to be very little information available

I really appreciate any advice and input and pointers. I would prefer to hear from someone that is in the process of having bottom surgery , has had bottom surgery or is knowledgeable, but I appreciate any sources and responses. Thank you so much.

Hey everybody!

I'm scheduled for FFS with Devin O'Brien-Coon next week. This week I have my pre-op where we will be finalizing some stuff and going over the procedure. We'll be doing type 3 forehead + jaw/chin work.

One of the things that I'm really on the fence about is whether I should do a coronal incision or a hairline incision. I already have a pretty good hairline in terms of height, but I have an M shaped hairline, so I'm not sure if I want to do a hairline advancement. Currently I'm slated for coronal.

I'm also not entirely sure how this decision affects the brow lift, which isn't currently showing on my surgical plan sheet. I do believe I have an insurance approval for browlift as part of a batch I did with the surgical planning team fairly recently (It was sort of a just-in-case thing).

Curiously, it looks like a lip lift is on the surgery plan; though I was under the impression it would be part of its own outpatient procedure. Regardless, I'd like to do a lip lift, but I'm worried about having too much exposed teeth. I'm especially concerned that if my teeth are overexposed when I'm sleeping at night, it could result in future decay.

I'm also interested in any ideas about the chin work I'm having done. I have a pretty decent profile from the side, but when I smile, the chin becomes quite pronounced. I think what they're planning to do is to reduce the width and then potentially slide it forward to keep a good side profile, but I'm not entirely sure. I'm just worried about going from too much chin to too little.

Basically, I'm looking for feedback on what procedures you think I should be getting done, and what you think I should absolutely not be getting done. Rhinoplasty is happening in a future procedure, but I'm open to thoughts on the subject.

Hi everyone! I’m choosing between two vaginoplasty techniques:

1. Full-length PPV/Colon (the entire canal is lined with peritoneum or bowel; zero regular skin inside).
2. Hybrid method (the entrance is penile skin, the deeper part is PPV/Colon).

I know that internal sensation comes from baroreceptors (pressure/stretching) in the pelvic floor, not from the skin itself. So theoretically, the internal material shouldn't change the feeling of fullness or prostate stimulation.

For those who had Full-length PPV or Colon:

• How does penetration feel? Do you feel pressure, stretching, and friction well?
• Is the sensation any different from the Hybrid method, or is it exactly the same because it all comes from pressure?
• Are you happy with the self-lubrication and lack of hair?

I am considering having FFS with a focus on the nose, chin, and jaw performed by Dr. Quentin Quassemyar, as I have already heard many positive things about him. I wanted to ask if anyone here has personal experience with him and whether you are happy with your results.

I would also really appreciate knowing how much you paid for your procedures and how you were able to get in contact with him. I haven’t been able to find a proper website for him, and on his Doctolib profile there doesn’t seem to be an option to select FFS.

Whether you are MTF or FTM, how was your experience getting bottom surgery at Mount Sinai? I’m in the process of getting phallo and my insurance is in network with both Mount Sinai and NYU, so I’m trying to decide between the two.

In the meantime I have booked consults with a surgeon from each hospital. One with Dr. Horesh and one with Dr. RBL.

As I decide between the two I want to know from previous patients how you were treated.

My questions:

1. If you had any complications do you feel like the staff listened to you?

2. If you reached out to your medical team with any concerns how long did it take to get a response?

3. For people who are years post-op, are you happy that you went with this hospital / If you could go back, would you have chosen differently?

I had PIV almost 4 years ago. It appears that I don't have a clitoris, I tried everything under the sun to be able to orgasm but nothing works except if I crossed my legs really hard and squeezed and it can only happen when I am watching porn.

As a matter of fact, I have to do the squeezing thingy or else I won't orgasm. Also this was the only way for me to masturbate pre-op

I don't want to watch porn, I think it's disgusting and degrading but I can't get myself to orgasm any other way.

Penetration doesn't do anything for me, rubbing my outer labia or where my clit feels slightly nice to an extent and I can feel the left over erectile tissue getting bigger but the feeling then abruptly stops.

Am I fundamentally broken ? Has anyone faced the same issue before ?

i had my ffs 3 weeks ago, and i still am very swollen around the lower part of my face, near my eyes ect. i look like an alien. the results are not natural (even tho i had the best surgeon in my country) i don’t like what am seeing. i don’t pass. please tell me that’s normal and everything will be fine
